Who Qualifies for Hospice Care?
Hospice care is available when:

A physician certifies that a person has a life expectancy of six months or less if the illness follows its expected course

The patient chooses comfort care over curative treatment

The patient requires symptom management and supportive care
It's important to know: Patients can still receive treatment for pain and symptom relief while in hospice care. You can also choose to leave hospice at any time if your condition improves or if you decide to pursue curative treatment.
Coverage and Eligibility
Medicare Hospice Benefit: Most hospice care is fully covered by Medicare Part A, including all services related to your hospice diagnosis.
(Pending) Veterans Benefits: Veterans may be eligible for hospice care through the VA, and we specialize in coordinating benefits and honoring the unique needs of those who served.

Our goal is to help you remain in the most comfortable, familiar environment possible while receiving expert medical care.
What to Expect When Starting Hospice
Initial Assessment Within 24-48 hours of admission, a nurse will visit to:

Create your personalized care plan

Answer questions and address concerns
Regular Visits: Your nurse will visit at least twice weekly, with additional visits based on your needs. Other team members (social worker, aide, chaplain) visit according to your care plan.
Communication: You'll have a primary nurse who coordinates your care. That nurse is your main point of contact, but our entire team is available 24/7 by phone.
Care Plan Updates: Your care plan is reviewed regularly and updated as your needs change. You and your family are always involved in care decisions.
