Skilled Homebound Rehabilitation — Coming Soon

American Veterans Hospice is expanding our continuum of care to better serve Veterans and seniors.
American Veterans Home Health will soon provide skilled medical care and rehabilitation services for patients who are homebound and need treatment in their homes to recover safely.

Our goal is simple:
Restore strength, prevent hospitalizations, and support independence — without leaving home.

Who Qualifies for Home Health?

Patients may be eligible for home health if they:

  • Are homebound (it takes considerable effort to leave home)

  • Need skilled nursing or rehabilitation therapy

  • Recently had a hospitalization, surgery, or health decline

  • Need help managing a chronic condition safely at home

  • Require therapy to regain mobility, balance, speech, or independence

Home health helps patients recover while staying in the comfort and safety of their home.

Home Health Services Coming Soon

Once licensed and approved, American Veterans Home Health will offer:

Skilled Nursing

  • Wound care

  • Medication management

  • Teaching on new diagnoses

  • IV care, catheter care

  • Monitoring vitals + chronic condition management

Home-Based Rehabilitation

Physical Therapy (PT)

  • Strengthening

  • Gait training

  • Fall prevention

  • Improving mobility and safety

Occupational Therapy (OT)

  • ADL training (bathing, grooming, dressing)

  • Adaptive equipment training

  • Home safety modifications

Speech Therapy (ST)

  • Swallowing therapy

  • Communication and cognitive rehab

Additional Support

  • Medical Social Worker (MSW)

  • Home Health Aide for limited personal care

All services are doctor-ordered and follow a customized Plan of Care.

What Home Health Is NOT

Home health is not hospice and does not require a terminal diagnosis.

  • Home Health = rehabilitation + medical stabilization

  • Hospice = comfort care without curative treatment

If a patient qualifies for both at different times, we help families understand the right level of care.
